这天医馆“从天而降”一位不速来客——慕容锁柱，号称自己有金刚不坏之体五雷不伤之命，千百年来就是死不了，来到医馆一心求死。众人开始以为这是开玩笑的，但渐渐发现，眼前这位奇人说的句句属实。此人厌倦了一眼望不到头的人生，看着身边最亲最近的人一个个离自己远去，见惯了世间的悲欢喜乐，感觉人生毫无意义，威胁医馆不杀他，他就誓不罢休。医馆众人秉着满足“客户”所有要求的原则，开始了各种行动，但不论文的还是武的，医馆众人都不是他的对手，几番交锋下来，慕容锁柱没死，医馆众人快疯了。李世真眼看着医馆众人陷入疯魔状态，只好亲自出马，动用智慧连哄带骗，向慕容锁柱描绘了千年之后的繁荣景象，慕容锁柱对千年之后繁荣富强万民协乐的景象所感染，点燃了慕容锁柱活下去希望，两人定下千年之约，慕容锁柱找到活下去的动力心满意足的离开。
